Summary of Position:
The LVN provides basic nursing care to residents within Skilled Nursing, utilizing established policies and procedures, ensuring safe and effective care. Services are provided based on the ages of the population served; Adult through geriatric - 65 years or older and in collaboration with the multidisciplinary care team. Assists the RN to assess and plan care by collecting resident care data through observation and interview. At the direction of the RN and in collaboration with the multidisciplinary care team implements and evaluates care. The LVN demonstrates the skill necessary to meet the individualized needs of all residents served in Skilled Nursing.
Qualifications
Education:
• Completion of accredited Vocational or Practical Nursing program
Experience:
• Previous experience in long term care preferred
• Sufficient computer skills are required to complete an online application and the pre-employment/annual (LMS)requirements
• In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred
Certifications/Licensure:
• Current California LVN license
• Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider)

Essential Functions
1. Provide consistently exceptional care at all times.
2. Demonstrates understanding of the role of the LVN as part of the care team. Communicates with team members to ensure optimum resident care.
3. Recognizes need for purpose vs. task when caring for residents.
4. Under the supervision of the RN, provides basic nursing care and evaluates outcomes.
5. Performs basic nursing procedures including thermal application, non-sterile soaks, and application of simple dressings, perineal care, and orthopedic splints.
6. Follows all appropriate infection prevention measures in place in the SNF at all times.
7. Collects resident care data at designated and appropriate intervals on all assigned residents, recognizing disturbed physiology and abnormal parameters. Reports finding to RN.
8. Evaluates psychosocial needs of residents, expressing interest in their progress and comfort and prepares them for disposition. Reports concerns to RN.
9. Monitors resident reaction and compliance with physician orders. Reports any untoward change in resident condition to charge nurse.
10. Recognizes unusual conditions outside of his/her expertise and seeks guidance of manager or RN.
11. Conducts hourly rounds on unit assigned every shift.
12. Medication administration is safe, accurate and timely, utilizing 5 rights of medication administration
13. Follows all safety precautions related to resident ID, order confirmation, calculations, and double-checking dose accuracy if applicable.
14. Follows procedure when removing medication from floor.
15. Documents all medication intervention to include rationale for PRN medications.
16. Reports all Adverse Drug Events (ADE) and medication errors according to policy.
17. Assists physicians with examinations, procedures and other processes related to direct resident care.
18. Keeps resident/family informed of progress, procedures, and disposition. Focuses on customer service needs.

26. Monitors and ensures accuracy of documentation in resident medical record. All aspects of resident care are accurately and timely documented. Completes all end of shift documentation, including but not limited to behavior monitoring and alert charting for appropriate residents.
27. Operates equipment and performs job related duties in a safe manner. Ensures proper functioning of equipment and follows procedure when equipment malfunctions.
28. The staff member is able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the residents served. The individual demonstrates knowledge of the principles of growth and development over the life span and possesses the ability to identify each resident's age-specific needs. All competencies are based on ages served
29. Demonstrates a working knowledge of regulatory and licensing standards and ensures that practice is compliant with standards.
30. Responds to the needs of the department by performing other duties, as necessary.
