Colorado Permanente Medical Group is searching for a board certified geriatric trained physician to help start a clinic within our medical group to help care for our adult patients with high needs.  Geriatric fellowship preferred or completion of the UCLA Intensive Course in Geriatric Medicine & Board Review, or similar training, should be completed within the first year of hire.

Complete Care Physicians specialize in the care of adult members with high needs due to a complex array of medical, social, psychological, and behavioral factors.  These Primary Care doctors focus on matching care to the needs of the individual patient using an interdisciplinary approach focused on increased continuity access, panel management, and care coordination.    A comprehensive team of healthcare professionals will ensure the well-being of members by handling a range of services including on-site and virtual primary care, chronic and acute care management, coordination with relevant specialists, and after-hours call coverage. Experience and/or formal training in complex care, geriatrics, and team-based approaches is preferred. Due to the high patient complexity, target panel size for Complete Care physicians will be ~600 members per 1.0 FTE. 

This is a 5 days a week, including Saturdays, based on 1.0 FTE with some of that work encompassing virtual call coverage from the hours of 8AM to 8PM.

  • Pay range: minimum $294,417.00 - 304,211.00. *Salary will be based on years of experience and FTE. 

Employment Requirement: Colorado Permanente Medical Group, P.C. staff must be fully vaccinated against COVID-19 or have established a qualified medical or religious, exemption by September 17, 2021. All persons offered a position will be required to provide valid proof of vaccination or qualifying exemption prior to starting employment.

     Major Responsibilities/Essential Functions - In order of importance, list primary job responsibilities.     

  • Provide comprehensive medical services for members in a Primary Care setting on a continuing basis:  Treat internal disorders, such as hypertension, heart disease, diabetes, and problems of the lung, brain, kidney, and gastrointestinal tract.
  • Analyze records, reports, test results, or examination information to diagnose medical conditions of the patient.
  • Prescribe or administer medication, therapy, and other specialized medical care to treat or prevent illness, disease, or injury.
  • Provide and manage long-term, comprehensive medical care, including diagnosis and nonsurgical treatment of diseases for adult patients in an office setting.
  • Manage and treat common health problems, such as infections, influenza and pneumonia as well as serious, chronic, and complex illnesses in adults and the elderly.
  • Monitor patients' conditions and progress and reevaluate treatments as necessary. Collect, record, and maintain patient information such as medical history, reports, and examination results.
  • Make diagnoses when different illnesses occur together or in situations where the diagnosis may be obscure.
  • Explain procedures and discuss test results or prescribed treatments with patients.
  • Advise patients and community members concerning diet, activity, hygiene, and disease prevention.
  • Utilizes computer to access patient records and other clinical information to include accurate coding.
  • Inoculates and vaccinates patients to immunize patients from communicable diseases.
  • Performs office procedures commensurate with scope of practice (such s minor office surgery, gyn, office orthopedics, etc.).
  • Refers patients to medical or surgical specialties for consultant services when necessary for patient's well-being.
  • Works as a part of a comprehensive care team that involves MA/LPNs,RNs social work and care coordinators.
  • Will care for a smaller panel of patients than would typically be done in an outpatient general Primary Care setting due to the highly complex needs of members eligible for the Complete Care Clinic.
  • Sees patients in a clinic setting with daily schedule involving meeting patient visit standards and must be flexible to attend to additional walk-ins and acutely ill or injured patients.  
  • Answers and follows through promptly on all telephone and email messages.  
  • Must be able to participate in holiday, after-hours and weekend schedule rotations, which could include travel to different medical office locations.  
  • Will be part of a team responsible for all outpatient care for their population of empaneled patients.  A small panel size will allow for more immediate access to continuity care when needed as well as providing usual routine comprehensive outpatient care services including prevention and wellness visits.   
  • Active participant in interdisciplinary team meetings, case reviews, and continuing education sessions.
