Staff Pharmacist - Per Diem
Sky Lake Medical Center
Klamath Falls, OR 

As a per diem, CPS Solutions, LLC Staff Pharmacist at Sky Lake Medical Center, you will fill orders with accuracy; check technician-prepared products before release; maintain security of pharmacy and controlled substances; enter orders and patient information with accuracy; aid in the training of new staff; update the Director about the daily activities of the pharmacy; and aid the Director in maintaining the pharmacy.  

Pay Rate:  $55 to $65 per hour, dependent on experience 

Schedule: This per diem, position will provide coverage to cover PTO, weekends and holidays as needed. There are no regularly scheduled hours for this position.  

About the Facility: Sky Lakes Medical Center is a 176-bed, non-for-profit, internationally accredited, acute care teaching hospital serving more than 80,000 people in Klamath and Lake counties in Oregon and Modoc and Siskiyou counties in northern California. As the only hospital in a 10,000 square mile area, Sky Lakes combines small town charm with big-city medical expertise. CPS Solutions, LLC has partnered with the hospital to manage the day to day operations of this pharmacy. This position is an employee of CPS Solutions, LLC. 

About the Pharmacy:  Pharmacy hours are Monday through Friday from 8:30am-5:00pm. 

Required Qualifications: 

  • Pharmacy degree (BS or PharmD) from an ACPE accredited school of pharmacy; PharmD preferred  
  • Current Oregon Pharmacist license in good standing  
  • Successful completion of a Pharmacy Practice residency program strongly preferred 
  • Experience as a pharmacist in a hospital setting preferred
  • Ability to work independently, set priorities, make critical decisions, and respond to client concerns
  • Demonstrated accuracy and timeliness in a work setting  
  • Experience using computer applications including word processing, spreadsheets, and pharmacy information systems  
  • Effective communication skills; verbal and written   

Hospital Requirements: 

  • (PPD) TB Skin Test - Proof of negative TB skin test within the last 12 months  
  • Fingerprinting 
  • (MMR) Measles, Mumps and Rubella or A Blood Titer proving immunity 
  • Varicella - (2) documented doses or A Blood Titer proving immunity 
  • Hepatitis B3 Series or declination
  • (Flu) Influenza - required for hires between October 1st - April 30th 
  • COVID-full vaccination
