  • POSITION SUMMARY
    • The Anticoagulation Clinical (ACC) Pharmacist is responsible for managing anticoagulation therapy in our outpatient clinic under a collaborative practice agreement. The ACC Pharmacist works directly with patients to maximize the therapeutic benefit of anticoagulation therapy while minimizing the risk of thromboembolic and hemorrhagic events utilizing evidence-based medicine. The ACC Pharmacist demonstrates professionalism and collaborates with providers, nurses, and other health care professionals to achieve established goals of therapy. The ACC Pharmacist pursues continual professional development in anticoagulation and applies the knowledge to provide individualized patient care. The ACC Pharmacist documents care in the electronic medical record.
  • POSITION QUALIFICATIONS
    • Education and Experience:
        • Doctorate of Pharmacy degree or Bachelor of Science. Graduate of an accredited School of Pharmacy.
        • Prefer previous working experience as a clinical pharmacist in anticoagulation or closely related field.
        • Prefer completion of ASHP accredited residency program with anticoagulation component or previous experience as an anticoagulation provider in a clinic setting.
    • License/Certificates:
        • Current Pharmacist certification or licensure in the State of Minnesota 
        • Current Basic Life Support (BLS) Certificate or obtain within 45 days of hire
    • Special Skills and Aptitudes:
        • Excellent problem solving and communications skills (verbal and written)
        • High level of clinical skills and pharmacotherapy knowledge 
        • Strong patient care orientation
        • Demonstrates innovation, initiative, and persistence in working collaboratively with multidisciplinary healthcare professionals 
        • Exemplifies self-direction with good organizational and analytical skills
        • Knowledge of clinical pharmacy practice
  • ESSENTIAL RESPONSIBILITIES
      • Work under a Collaborative Practice Agreement (CPA) which allows the anticoagulation pharmacist to monitor and adjust anticoagulation therapy per a specified protocol.
      • Obtain and evaluate current and past medical history, medication changes (including prescription, OTC, and dietary supplements) and lifestyle changes that could impact anticoagulation therapy. 
      • Conduct face-to-face, telephone, and telehealth patient visits. 
      • Utilize point-of-care testing to measure and evaluate the international normalized ratio (INR) and provide patients anticoagulation dosing and recheck recommendations. 
      • Order and evaluate laboratory tests (e.g INR, CBC, hemoglobin/hematocrit (Hgb/Hct), serum creatinine (SCr), etc.) as outlined in the Collaborative Practice Agreement.
      • Monitor patients for signs/symptoms of bleeding or thromboembolic events. 
      • Document patient visits in the electronic medical record and communicate appropriately with the primary care provider. 
      • Provide education to the patient and caregivers regarding anticoagulation treatment indication, medication information, medication adherence, anticoagulation monitoring, safety considerations, drug-drug/food-drug interactions, etc.
      • Schedule follow-up appointments or supervise support staff in scheduling.
      • Assist in development and coordination of perioperative anticoagulation therapy plans for medical or dental procedures with specialist/surgeon and primary care provider.
      • Electronically prescribe or phone in prescriptions under the primacy care provider.
      • Collaborate with others to resolve concerns related to patient care, communication, policies, and resources.
      • Be a resource for providers, nurses, or other health care staff regarding anticoagulation drug therapy. Provide or coordinate education as needed.
      • Maintain current knowledge of pharmacotherapy, pathophysiology, and evidence-based guidelines and implement them into daily practice. 
      • Identify opportunities to expand and improve anticoagulation services throughout the organization. 
      • Actively participate in interdepartmental committees, meetings, and teams as needed.
      • Actively participate in department planning, goal setting, and implementation of those goals or plans.
      • Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. 
      • Ensure the maintenance and proper operation of equipment and supplies in the work areas. 
      • Assist the department manager with other responsibilities in accordance with the organization's mission, policy, and applicable laws.
      • May supervise at least one to two (1-2) members of the Pharmacy Department staff daily.
      • May precept PharmD students or pharmacy residents.
