A Day in the Life of a Palliative Care Coordinator

The Palliative Care Coordinator is a pivotal nursing role focused on the development and management of palliative care services, ensuring the program aligns with regulatory standards and operational goals. This role involves direct patient interaction from initial diagnosis through the entire care continuum, involving assessing needs, managing symptoms, providing essential information, and supporting patients and families. Challenges include maintaining high-quality care while coordinating with multi-disciplinary teams including physicians, medical staff, and external partners to optimize patient outcomes. Collaboration is key, as the coordinator acts as a liaison among various stakeholders and advocates for patients in interactions with insurance entities and regulatory bodies. The structure includes developing policies, overseeing staff training, and continuously improving program quality based on the latest palliative care trends and regulations.

From coordinating discharge plans and arranging follow-up care to addressing social determinants of health, our team ensures a smooth transition from hospital to home, rehab, or other care facilities. Qualifications

Education and Experience Required
  • Two years of experience in Palliative Care
  • Current Minnesota RN license
  • American Heart Association BLS within 30 days of hire
  • 5 year's experience in acute care
  • Palliative Care Certification within 3 years of hire
  • Valid Minnesota driver's license
  • Bachelor of science/art in nursing
Preferred
  • Master's degree in Health Care related field (Nursing, Social Work, Health Care Administration, etc.)
Compensation and Benefits

The pay range listed above is the estimated starting pay range for this role. Actual pay rate offered depends on many factors including each individual employee's experience level, preferred education/certifications, along with market conditions.
