Description:

We are looking for an independent and adaptable Social Worker to join the Care Management Team. The social work team assists patients and families in coping with illness, optimizing the patient's self-care abilities, and supporting patient's rights to make choices. The ideal candidate will excel at promoting teamwork and will demonstrate a high degree of professionalism in a fast-paced work environment. The social work team supports patients across the continuum of care with exception care coordination and effective collaboration skills.

Qualifications:

  • Graduate of an accredited program of Social Work or a Baccalaureate degree in Social Work from a nationally or regionally accredited college or university. MSW preferred.
  • Current Minnesota Social Workers license preferred. If not licensed, must obtain licensed prior to start date.
  • Three years' experience as a social worker in the acute care setting or related setting is preferred.
  • Must hold a valid Minnesota driver's license.

Responsibilities and Skills:

  • Complete a comprehensive assessment of the patient's physical, psychological, social, environmental, financial, and functional status.
  • Coordinate care for medical services, mental health needs, long term care, substance abuse, and other services.
  • Work closely with internal and external partners to improve outcomes for patients.
  • Support a culture of continuous improvement through quality monitoring, regulatory compliance, risk management and performance improvement activities.
  • Monitor patient progress toward goal achievement and patient outcomes.
