Come join our Venice Family Clinic as a Health Educator and make a positive difference in others' lives. The Diabetes Educator works within a multi-disciplinary healthcare team in the delivery of comprehensive services for patients diagnosed with diabetes and chronic illness in a primary care setting. Acting as the primary contact person for the family, the Diabetes Educator assists the healthcare team in the development and implementation of a health care plan tailored to the needs of the patient and the patient's family in order to promote continuity of care, improve the patient's health, and ultimately reduce the rate and severity of diabetes-related complications.

This position will be based at our Clinic in Inglewood (1091 S. La Brea, Inglewood CA 90301), but will also work at our Gardena Health Center (742 W. Gardena Blvd, Gardena CA 90247) and South Bay Family Health Care (2114 Artesia Blvd, Redondo Beach, CA 90278) locations.

-Salary offers are determined based on various factors including, but not limited to, qualifications, experience, and equity. The full salary range for this position is $30.56 - $60.82 hourly. The budgeted salary range that the University reasonably expects to pay for this position is about $30.56 - $40.00 Hourly.-

Qualifications

Required:

Master's degree in Public Health, Nursing, Nutrition or a related field OR a Bachelor's degree in Health or a related field and at two years of experience providing health education services in a public health or primary health care organization.

Working knowledge of health education issues and programs including nutrition, lifestyle, and behavioral problems.

Working knowledge of diabetes and strategies to improve patient adherence to medical treatment plans

Bilingual English/Spanish language skills.

Ability to work effectively as a member of an interdisciplinary team, and establish and maintain cooperative working relationships.

Knowledge of the Latino culture and diet.

Excellent communication and organizational skills, and the ability to effectively communicate with a wide variety of people of various educational and socio-economic backgrounds.

Ability to function efficiently and independently, and to plan and prioritize daily work activities to meet clinic goals and objectives.

Demonstrated proficiency in using Microsoft applications such as Word, Excel, and Access.

Ability to travel between clinics and community organizations to conduct health education programs, presentations, services, and outreach.
