The Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) is the state agency that focuses solely on policies, programs, and services for people with mental health challenges, substance use disorders, intellectual and developmental disabilities, or any combination of these. As Georgia's public safety net, DBHDD operates six field offices and five hospitals throughout the state within three program areas: behavioral health, developmental disabilities and hospital services.

The main office for DBHDD is located in downtown Atlanta.

Job Description, Responsibilities, Standards, and Qualifications

**THIS POSITION IS NOT A HOSPITAL POSITION. IT IS A TELEWORK POSITION AND REQUIRES EXTENSIVE TRAVEL THROUGHOUT THE REGION**

Coverage area for this position includes but not limited to the following counties: Richmond County

HOURS: Monday-Friday 8am-5pm ; no weekends, no overtime, state holidays off

Assess healthcare needs of individuals with intellectual/developmental and behavioral related disabilities for supports to be provided in natural home, residential living and/or community based settings.

Assist in determination of intensity of supports needed and offer choice in providers available to provide level of care and coordination needed.

Make recommendations to be carried out by interdisciplinary team regarding identified clinically necessary services and supports for the individual. Review provider records for delivery of supports identified within best healthcare practices. May advise individuals and providers on health maintenance and disease prevention or provide coaching and training as requested.

May assist in the development of person-centered care plans and health-related ISP goals. Provide education to individual and family.

Review all healthcare assessments and other medical records, including the HRST and the MAR for compliance and accuracy.

Consult and coordinate with health care team members to assess, plan, implement and evaluate health care plans.

Write accurate, detailed reports and upload to electronic file.

May be asked to observe nurses and visit individuals' homes to ensure that proper nursing care is provided.

Qualifications - External

Bachelor's degree in nursing from an accredited college or university OR Associate's degree in nursing from an accredited college or university AND current Georgia license as a Registered Professional Nurse AND any related professional work experience.

Preferred Qualifications - External

Current Georgia license to practice nursing as a Registered Nurse

Experience working with developmentally disabled people
