Summary The incumbent work is of considerable difficulty and responsibility performed under minimal supervision. The WHCN LPN exercises independent technical judgments and comprehensive working knowledge to provide women Veterans seamless access to comprehensive health care in primary care, gender specific care and behavioral health. The LPN is responsible and accountable for direct and indirect patient care, quality nursing care management to a culturally diverse, female Veterans population. Responsibilities Duties include but are not limited to: Provides a full range of LPN/LVN care to patients related to the WVP duties. Has the ability to work independently and provide modalities to Veterans that have multiple complex medical and or emotional conditions. Demonstrates adherence to VA and Nursing policies/procedures and standards and maintains competencies consistent with role responsibilities. Assists the WVPM, Deputy WVPM, Women Health Medical Director (WHMD) and other WVP staff in proactively identifying and addresses the veteran's needs and can readily recognize the subtler signs & symptoms of changes in the veteran's condition and takes appropriate action to provide efficient case management. Participates in the development of educational material for providing veteran, family and caregiver teaching regarding WVP, cancer screening and MCC. Teaches and documents health concepts to WH veteran, including learning needs and readiness to learn. Is self-directed in seeking out and participating in learning opportunities for the veteran. Demonstrating skill/knowledge/ability to perform treatments, procedures and the use and reprocessing of reusable medical equipment appropriate to assigned duties. Maintains current licensure and BCLS. Accurately reports pertinent, signs, symptoms, conditions, and behavior of patients to the WVPM or WHMD as deemed necessary by the patient's condition and needs. Accurately and timely records all patient data, and education into the medical records. Actively participates (as directed) in interdisciplinary team planning sessions by offering observations and suggestions. Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language: Must be proficient in spoken and written English as required by 38 U. S. C. 7402(d) and 7407(d). Education: Must be a graduate of practical or vocational nursing approved by the appropriate State accrediting agency and/or the National League for Nursing at the time the program was completed by the applicant. License: Must have a full, current, active and unrestricted license as a graduate licensed practical or vocational nurse in a State, Territory, or Commonwealth (i. e., Puerto Rico) of the United States or District of Columbia. Grade Determinations: GS-7: Experience: completion of at least one year of additional qualifying experience at the GS-6 level or equivalent, fully meeting all the performance requirements for the GS-6 LPN/LVN. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (KSAs must be clearly reflected in resume under description of duties in order to be found qualified for this position/grade level). Knowledge and skills necessary to carry out more specialized patient care duties, including assignments in selected, more complex inpatient and/or outpatient care settings. Knowledge base promotes the incorporation of current patient care data in actively contributing to the development of the patient's overall health plan and in carrying out assigned nursing interventions. Positions at this grade level require broader knowledge and skills because supervision is more general and employees are expected to be involved to a greater degree in the development and modification of treatment plans. Ability to assist RN and/or MD/DO to appropriately prioritize overall patient care needs and adjust plans for care delivery. Makes astute observations of subtle, less obvious changes, both physical and emotional, in patient's condition, and accurately and thoroughly documents care delivered and patient's response to care that could require immediate modification of the patient's care plan. Appropriately seeks assistance and guidance by informing RN or MD/DO of changes in patient's condition requiring higher-level intervention. At this grade level, the LPN/LVN is expected to participate to a greater degree in prioritizing, and, as appropriate,
