Your Specific Responsibilities:

This is highly independent and responsible professional position. This position manages and coordinates activities/deliverables for Healthiest Weight. This position will work under moderate supervision by the Executive Community Health Nursing Director.

Serves as Healthiest Weight Coordinator for Holmes and Washington CHD's ensuring all work plan activities are completed in a timely manner. Submits monthly report and attends any required meetings or trainings. Promotes healthy lifestyle choices within Holmes/Washington County communities. Provides health education and screenings to the community, clients, and general population.

Ensures that activities and budget management of the programs are followed by grant guidelines.

Responsible for quarterly reporting of all activities for Holmes and Washington Counties completed in connection with the Healthiest Weight workplan.

Supports local community project initiatives as directed by supervisor.

Attends any required meetings or trainings related to Incident Command System, disaster response, staff meetings, and department meetings.

Utilization of Health Management System (HMS) for computer input. Responsible for inputting Employee Activity Records (EARs) into HMS at Holmes/Washington by Friday of each week.

Performs other duties and responsibilities as assigned by supervisor or requested by DOH.

Required Knowledge, Skills, and Abilities:

Knowledge of the principles and practices of health education.

Knowledge of a variety of health specialty areas.

Knowledge of nursing and public health principles, practices, and techniques.

Knowledge of human anatomy and physiology.

Knowledge of community resources.

Skill in operating a variety of audiovisual and computer equipment.

Ability to plan and coordinate the development and implementation of health promotion and public health education programs and projects.

Ability to prepare a wide variety of written material such as reports, memoranda, pamphlets, curricula, lesson plans and grant requests.

Ability to collect and analyze data.

Ability to provide preventive health care and treatment to groups.

Ability to work with patients having a variety of physical problems.

Ability to determine work priorities, assign work, and ensure proper completion of work assignments.

Ability to plan, organize and coordinate work assignments.

Ability to communicate effectively.

Ability to establish and maintain effective working relationships with others.

Ability to teach basic health care.

Qualifications:

Required - Licensed Registered Nurse, Florida Statutes 464

Required - Valid Driver's License

Required - High School Diploma or equivalent
