The Care at Home Registered Nurse remotely monitors and navigates patients and families across the continuum of the Medically Home phases of care. The Care at Home Registered Nurse performs assessments, monitoring, planning and interventions for acute care patient populations in the home while using in-home technology and protocols in collaboration with other members of the health care team.

**A caregiver in this position works three12 hour shifts, with rotating weekends and holidays. This position is** **not** **a work from home role, and is located out of Vero Beach, FL.**
Minimum qualifications for a caregiver in this role are:
Minimum one year current acute care RN experience in a hospital setting.
Graduate from an accredited school of professional nursing.
Current state licensure as Registered Nurse (RN).
Basic Life Support (BLS) through American Heart Association (AHA)
Preferred qualifications for the ideal caregiver are:
Three years of inpatient RN experience.
Strong verbal and visual assessment abilities as this position will be providing virtual care to Medical/Surgical level patients.
Can critically think and problem solve for patients' best interests.

**Physical Requirements:**
Requires full range of body motion including handling and lifting patient, manual and finger dexterity, and eye-hand coordination.
Requires standing and walking for extended periods of time.
Requires corrected vision and hearing to normal range.
Requires working under stressful conditions and irregular hours.
Exposure to communicable diseases and/or body fluids.
Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
Physical Demand requirements are in excess of those for Light Work.
