About the Opportunity
We are seeking a Full-Time Registered Nurse (RN) to join our growing team. This RN will play a key role in providing skilled Medicare home health visits and assisting with the case management of Private Duty (non-skilled) services to ensure seamless coordination of care across service lines.
The RN will work collaboratively with the interdisciplinary team, physicians, patients, and families to develop and implement individualized plans of care. This position serves Hillsborough County and supports ongoing clinical growth and quality initiatives within the agency.
Responsibilities
- Provide high-quality, patient-centered nursing care while ensuring safety in the home environment
- Conduct skilled Medicare visits including assessments, wound care, medication management, IV therapy, and other skilled services
- Assist in overseeing and supporting Private Duty case management, ensuring clear communication and continuity of care
- Assess, plan, implement, and evaluate patient care in collaboration with the interdisciplinary team
- Educate patients and caregivers on disease processes, medication management, and prevention of complications
- Collaborate with physicians and healthcare professionals to optimize patient outcomes
- Complete all clinical documentation accurately and in a timely manner
- Participate in QAPI activities and contribute to ongoing agency improvement initiatives
- Attend interdisciplinary team meetings as required
Qualifications
- Active Florida RN license
- Minimum 1 year of nursing experience (Home Health experience required)
- Strong clinical judgment and critical-thinking skills
- Ability to work independently with strong organizational skills
- Excellent communication and interpersonal skills
- Reliable transportation
- Commitment to high-quality patient care and collaborative team dynamics
Physical Demands
This role requires the ability to perform physical activities such as lifting, balancing, walking, stooping, and handling materials. The RN must be able to navigate diverse home settings and adapt to different environmental conditions.
