Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders and clinical findings
• Establish functional goals designed to improve mobility, strength, balance, endurance, and overall quality of life
• Provide skilled physical therapy interventions in accordance with the plan of care
• Assess patient progress and modify treatment plans as appropriate to achieve optimal outcomes
• Educate patients, family members, and caregivers on therapeutic exercises, fall prevention, safety techniques, and equipment use
• Complete required clinical documentation accurately and within established timeframes
• Coordinate care with physicians, nurses, Physical Therapy Assistants, and other healthcare professionals
• Supervise and direct Physical Therapy Assistants in accordance with state practice regulations
• Participate in patient discharge planning and recommendations for continued care and services
• Provide home health and/or outpatient therapy services as assigned

Qualifications:

• Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
• Must be licensed or registered by the applicable State without restrictions.
• One year of home health experience preferred.
• Must have knowledge of Medicare and Medicaid guidelines
• Must have a valid driver's license and reliable transportation.
• Must have a current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.

Background Screening Requirement: Employment in Florida is contingent upon completion of a Level 2 background screening in accordance with Florida AHCA requirements. Applicants will be required to complete fingerprinting and background screening through the AHCA Clearinghouse.
