Job Summary:

The Case Coordinator is responsible for incorporating several professional functions into the management of the total episode of care for patients on their assigned unit(s).  She/he uses independent judgment in connection with her/his managerial responsibilities.

Status: Full-time
Shift: 8 hour days
Hours: 9a - 5p

Job Essentials

• Requires a minimum of 3 years of case  mangagement experience in a hospital setting and/or home health care nursing experience

• Excellent documentation and communication skills.
• Ability to work flexibly.
• Proficiency in computer skills.

Educational Requirements

• BSN or proof of active enrollment in a BSN program.

Licensure/Registration/Certification

• NJ State RN  License
• BLS Certification
