Job Summary
Provides day-to-day leadership to social workers providing services to patients within the hospital, Emergency Department, Community Medical Centers, and Physician's Offices. Coordinates and provides social work for patient populations including pediatric, adolescent, adult, and geriatric patients and their family members or significant others to assist in meeting their social and emotional needs. Acts as a clinical resource for all social work staff. Serves as the expert for social work practice and issues for the organization.
Minimum QualificationsMinimum Education: Master's Degree in Social Work from an accredited college
Minimum Years of Experience: 3 Years of experience in hospital social work;
1 Year of previous experience where progressively responsible leadership was demonstrated.

Required Certifications/Licensure: Licensure as a Social Worker in the State of South Carolina

- The Manager of Case Management facilitates the delivery of patient centered care through the coordination of social work activities and the provision of direct patient care as needed. Demonstrates competence in the provision of developmentally appropriate care/treatment for the following patient population: infant, child, adolescent, adult, and geriatric.
- Leads and directs the daily operations of the medical social services staff.
- Updates social work policy and procedures to maintain accuracy and assure LMC compliance with national and regional standards.
- Actively seeks money for the Patients Emergency Care Fund with the Foundation, Volunteers Auxiliary, Administration, and other community groups that the Foundation would recommend.
- Functions as staff social worker as needed. Following duties are included:
- Accepts referrals by Physicians/Nurses/others for psychiatric assessments and discharge planning.
- Interviews patients/family members/significant others to assess their particular situations and advise them of appropriate community resources.
- Councils with patients/family members/significant others regarding patients social, emotional, and financial situation relating to care of the present medical situation. Explains clearly to them what the social worker will do to assists them in getting the most appropriate services for the patient. The social workers also must inform them what they must do to receive community services.
- Serves as a patient advocate by ensuring that all planned and needed services are received in a timely manner.
- Reassess patient's needs, progress, setbacks, and adjusts services to meet the changing needs of the patient and/or caregiver.
- Records significant events in the patient's medical chart.
- Functions as a patient advocate.
- Coordinates with other service areas to ensure proper patient care.
- The Manager of Case Management will embody those behaviors, philosophies, and values that serve to uphold the ideals and enhance the image of Lexington Medical Center (LMC).
- Models LMC philosophy, standards of care, standards of practice, policies, procedures, and protocols.
- Functions as professional role model for social workers.
- Demonstrates receptiveness to change and flexibility in meeting department needs.
- Maintains current knowledge base (attains 40 hours of educational hours annually).
- Identifies staff educational needs and implements measures to address needs.
- Abides by the National Association of Social Work Code of Ethics.
- Actively involved in discussion of unusual/complex cases and advises social workers/discharge planners with possible solutions.
- Promotes high standards of care and practice and holds staff and self-accountable.
- The Manager of Case Management will assess human resource needs and plan and evaluate employee activities to ensure safe, effective, and fiscally sound operations. Responsible for recruiting, hiring, and retaining of qualified staff, ensuring competence through orientation and education.
- Demonstrates ability to work with personnel in a manner that fosters positive employee development and performance.
- Assists with the development and adherence of personnel budget.
- Promotes an environment which provides opportunities for the continued professional development and broadened knowledge of staff.
- Monitors and evaluates employee performance to include probationary, annual, and disciplinary actions.
- Participates in the development, implementation, and evaluation of orientation programs for new employees.
- Develops and maintains accurate and efficient schedule for social work staff.
- Implements measures to assure productivity and effective use of human resources within the department.
- Coordinates obtaining students with local and regional Colleges of Social Work for field placement: interviewing, selecting, and coordinating with hospital orientation as well as social work agency visits, training students on interviewing, documentation, and correctly using community resources.
- Works with Director on staffing changes.
- Promotes and uses effective reward and recognition strategies.
- The Manager of Case Management selects, uses, and accounts for material resources with appropriate consideration for patient/staff needs, hospital policies, and procedures and budgetary constraints.
- Collaborates with Director in the development of department capital and operating budgets and supports departmental operations within budgets.
- Seek out new community resources.
- Responsible for maintaining up-to-date written information regarding community resources.
- The Manager of Case Management will communicate effectively throughout the organization.
- Keeps Director of Case Management and other HCTM informed of activities through oral or written communications.
- Represents department on various organization committees as assigned.
- Attends and serves on professional/civic service organizations as hospital representative.
- Efficiently uses electronic communication and other system supports for conveying information.
- Interacts with others in a manner which promotes effective problem solving and team management.
- The Manager of Case Management identifies opportunities for improvement of patient care, health care practice and interaction with health care team, and initiates and/or participates in development of action plans to achieve desired outcomes.
- Collaborates with Director to develop departmental goals, objectives, and continuous quality improvement plans.
- Implements policies and procedures for department operation to support goals.
- Reports progress toward goals periodically.
- Participates in the identification of customer needs and expectations.
- Uses available resources and appropriate administrative and organizational channels to facilitate the change process.
- Follows hospital guidelines, P&P regarding time and attendance.
- Performs all other duties as assigned by authorized personnel or as required in an emergency (e.g., fire or disasters).
