General Summary of Position
The MedStar Health Home Infusion Pharmacist plays a vital role in ensuring the safe effective and efficient preparation dispensing and monitoring of infusion therapies delivered to patients' homes. This position provides clinical oversight patient education and collaboration with healthcare providers while maintaining strict compliance with regulatory standards. In addition to optimizing patient outcomes through cost-effective pharmacotherapy the pharmacist contributes to the advancement of the profession by supporting experiential education for current and future healthcare practitioners and engaging in drug-related research initiatives.

Primary Duties and Responsibilities


  • Reviews and interprets physician orders for accuracy appropriateness and safety for home infusion in accordance with federal and state drug laws and other regulatory organizations. Communicate with physicians and nurses to ensure prompt clarification of orders that are unclear illegible or incomplete.
  • Reviews maintains and screens patient medication records and profiles for allergies drug interactions and therapeutic duplications while entering orders and/or verifying medications.
  • Provides drug information and clinical consultation to physicians nurses and caregivers. Works closely with physicians nurses case managers and insurance providers to coordinate care.
  • Handles prescription order entry to generate mix reports and labels needed for dispensing sterile medications including IV antibiotics chemotherapy TPN and pain management therapies.
  • Ensures compliance with USP <797>/<800> standards aseptic technique and complete yearly competencies.
  • Is familiar with the compounding and labeling processes.
  • Organizes daily operational duties and assist in the supervision of pharmacy technicians and other ancillary personnel involved in providing pharmaceutical care.
  • Maintains accurate and complete patient records including medication profiles and clinical notes.
  • Ensures compliance with state and federal pharmacy laws and accreditation standards.
  • Educate patients and caregivers on proper administration storage and handling of infusion medications.
  • Provide counseling on potential side effects drug interactions and adherence strategies. Monitor patient response to therapy compliance and recommend adjustments as needed.
  • Support patients via phone telehealth or in-person consultations as needed.
  • Is proficient with infusion pumps and be able to troubleshoot issues acquire and maintain pumps per policies and procedures.
  • Assists in discharge planning and transition of care from hospital to home.
  • Supports formulary management and cost-effective therapy selection.
  • Participates in quality improvement initiatives and protocol development. Follow up on any patient adverse reactions and/or complaints.
  • Is responsible for on call after hours for home infusion patient questions and emergencies.
  • Provides leadership coverage for the Director of Home Infusion during absences assuming delegated responsibility for operational management staff oversight and compliance activities.
  • Minimal Qualifications
    Education
    • Bachelor's degree Pharmacy required and
    • Doctoral degree Pharmacy (PharmD) required
    • One year of relevant education may be substituted for one year of required work experience.
    Experience
    • 1-2 years Minimum 2 years of experience in hospital pharmacy infusion or compounding pharmacy required
    Licenses and Certifications
    • PHARMD - Licensed Pharmacist in practicing state Upon Hire required and
    • Sterile Compounding Certification within 1 Year preferred
    • Pharmacist license in VA within 1 Year preferred
    • Pharmacist license in WDC within 1 Year preferred
    Knowledge Skills and Abilities
    • Strong clinical knowledge of infusion therapies and pharmacokinetics
    • Excellent communication organizational and problem-solving skills

    This position has a hiring range of

    USD $49.33 - USD $94.71 /Hr.
