 General Summary of Position
Serves as a member of the Case Management Team. Facilitates the delivery of quality cost effective patient-centered care from pre-admission through post-discharge timeframe. Ensures the care is designed to meet individualized patient outcomes. Monitors the care and services delivered to selected patient populations during the acute hospital stay working collaboratively with the multidisciplinary team both internal and external to the organization promotes effective case management and utilization of resources facilitating the continuum of care and works to achieve optimal clinical and resource outcomes for the acute and posthospital phases of care. 
Primary Duties and Responsibilities
Education
- Master's degree in Social Work from a school accredited by the Council on Social Work education required
 
- 1-2 years Experience in social work preferable in a hospital setting preferred
 - 1-2 years Experience in care/case management preferred
 
- LCSW-C - Licensed Certified Social Worker - Clinical in the District of Columbia or the State of Maryland depending on work location Upon Hire required or
 - LICSW - Licensed Independent Clinical Social Worker in the District of Columbia or the State of Maryland depending on work location Upon Hire required
 - CCM - Certified Case Manager Upon Hire preferred
 
- Diagnostic and problem-solving skills.
 - Must consistently demonstrate a positive attitude be solution oriented regarding departmental issues and exhibit behavior consistent with a team approach to accomplishing departmental outcomes.
 - Verbal and written communication skills.
 - Basic computer skills preferred.
 
This position has a hiring range of
USD $74,214.00 - USD $134,596.00 /Yr.
