Job Summary:
MedStar Health is looking for a MedStar Discharge Transfer Coordinator to join our team!
The MedStar Discharge Transfer Coordinator works with hospital case management, social work, and unit medical staff to provide services in coordinating discharge transportation to final destination. The position requires a forward-facing relationship with ambulance vendors in coordinating dedicated and street side transportation needs and is responsible for following up on safety issues and invoicing. The team will also be instrumental in participating in coordinating discharge during disaster management for the MedStar Health system.

Primary Duties:
- Serves as the primary contact for patient discharge transportation, collecting required demographic, insurance, and clinical information to coordinate transfers within the MedStar Health System.
- Collaborates with case managers, social workers, clinical staff, and transportation vendors to determine appropriate transport modes and ensure required forms and documentation are completed.
- Arranges and monitors ambulance dispatch based on patient acuity (BLS, ALS, Critical Care), utilizing vendor software and MedStar SafetyNet for tracking and issue resolution.
- Communicates with hospitals, long-term care facilities, physicians, and insurance partners to expedite approvals, resolve invoicing discrepancies, and ensure smooth transfer processes.
- Maintains confidentiality, adheres to regulatory and departmental standards, supports new initiatives, and demonstrates strong problem-solving and decision-making skills.
Qualifications:
- High School Diploma or GED (Required)
- On-the-job training (Preferred)
- Associate's degree (Preferred)
- 3-4 years case management or related field (Required)
License/Certifications:
- Emergency Medical Dispatcher (Current or prior registration) (Preferred)
- RN/LPN (Preferred)
- Medical Assistant (current or prior registration) (Preferred)
- Certified Nursing Assistant (current or prior) (Preferred)
This position has a hiring range of
USD $20.57 - USD $36.27 /Hr.
