Key Responsibilities
As a Cardiology Nurse Navigator Clinical Nurse, you will provide skilled, compassionate care to patients and families across a spectrum of needs—from routine to highly complex. As a key resource for patients throughout their cardiovascular care journey, some of the responsibilities include coordinating care, educating patients on diagnoses and treatment plans, managing follow-up appointments, reviewing test results, triaging patient concerns, and facilitating communication between patients, providers, and other members of the healthcare team. The Nurse Navigator helps ensure timely, seamless care while promoting positive patient outcomes and an exceptional patient experience. You will also assess evolving patient conditions, identify actual and potential health concerns, and develop appropriate care plans. Interventions may be delivered directly or coordinated through collaboration with other members of the nursing team.

Qualifications
- ADN, BSN, MSN, or CNL from an accredited School of Nursing required.
- Prior Clinical Nursing Experience preferred.
- Active MD RN License or Active Compact State RN and Basic Life Support for Healthcare providers required (AHA or American Red Cross).
This position has a hiring range of
USD $89,065.00 - USD $162,801.00 /Hr.
