General Summary of Position
The LPN Care Specialist plays a dual role, delivering both clinical and administrative support within Clinical Operations. Under the supervision of a Registered Nurse (RN), the LPN Care Specialist provides direct and indirect patient care while also assisting Clinical Operations Managers with daily operational tasks and compliance requirements. This hybrid position supports various aspects of Clinical Operations, including but not limited to workflow coordination, tracking mandatory requirements and documentation, updating patient portals, managing authorizations, and supporting program initiatives. The LPN Care Specialist may also assist with telehealth services, triage support, re-sequestration of MedStar-issued equipment and uniforms, and serve as a liaison between Operations and other departments across the agency. The role requires strong organizational skills, the ability to manage multiple priorities, and effective communication to guide appropriate escalation and resolution of operational issues. Additionally, the position supports benefit verification, as well as obtaining and managing authorizations as needed. By ensuring efficient clinical and non-clinical workflows, the LPN Care Specialist contributes to timely patient care, accurate documentation, and the overall success of Clinical Operations.


Primary Duties and Responsibilities


  • Contributes to the achievement of established goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Completes annual Code of Conduct, Compliance, and other mandatory training. Complies with governmental and accreditation regulations.
  • Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.
  • Performs day-to-day operations, especially in support of the Clinical Operations Manager and Regional and Assistant Regional Operations Directors (ARODs), by supporting daily routine tasks and duties as assigned. Applies guidance on policies and procedures through day-to-day interaction with field staff, escalates as needed, and problems solves to accomplish the task at hand.
  • Gathers, assembles, and reports out on necessary data that impacts operational and clinical outcomes for all paitents on teams by using appropriate tools and ongoing monitoring of agreed to metrics while raising relevant issues to higher-level management.
  • Provides telehealth visits as assigned, meeting defined productivity and expectations. Documents virtual encounters in accordance with regulatory and agency standards
  • Assists with implementation of associate engagement programs such as diversity/culture in local settings and promotes inclusion.
  • Performs patient calls related to experience concerns, vital sign alerts and potential high risk for hospitalization.
  • Serves as Clinical triage nurse to operations team (aka scheduling), patients, caregivers, and physicians to ensure timely care and appropriate escalation.
  • Supports processing data to ensure accurate and timely reporting.
  • Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations. Maintains compliance with Code of Conduct, regulatory requirements and safety standards
  • Provides good customer relations by maintaining open, timely communications with associates, interdepartmental, MedStar Entities, and customers at large. Interacts with vendors/suppliers to maintain service quality and mission.
  • Works collaboratively with all MHHC leaders to improve patient satisfaction and associate satisfaction.
  • Monitors patient flow from referral to admission to maintain optimal efficiency and safety. Communicate effectively with healthcare providers to relay patient delays in service. Provide patients with clear instructions during the triage process, ensuring a positive and informed experience
  • Communicates, in a timely manner, the status and outcome of the authorization process to all appropriate parties. Provides oversight to pending verifications/re-verifications. Communication may be made to Operations Units, Home Care Consultants, patients and referral sources. Maintains through the process of professional communication, a positive, effective working relationship with other members of the Agency.
  • Functions as a motivated, flexible member of the Referral Center team, demonstrating efficiency, professionalism, and a willingness to assure that the team has completed the day's assignments on time.
  • Obtains and verifies information required for billing. Communicates with insurance companies via internet, telephonic program or person to person to verify benefits and eligibility and to obtain authorization for home care services and supplies.
  • Participates in multidisciplinary quality and service improvement teams as appropriate. Participates in meetings, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate.

  • Minimal Qualifications
    Education

    • Graduation from an accredited program of Practical Nursing required

    Experience

    • 1-2 years Recent clinical experience in a home health or acute care setting required and
    • Home Health Experience preferred

    Licenses and Certifications

    • LPN - Licensed Practical Nurse - State Licensure Valid LPN license in the District of Columbia, Maryland or Virginia, or any combination as required based on work location(s) Upon Hire required and
    • DL - Valid State Driver's License in good standing with a dependable vehicle for transportation Upon Hire required and
    • CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required and
    • Additional unit/specialty certifications may vary by department or business unit.

    Knowledge, Skills, and Abilities

    • Verbal and written communication skills.
    • Basic computer skills preferred.


    This position has a hiring range of $22.04 - $38.41
