General Summary of Position
Provides diet and diabetes education services in the ambulatory setting during the perinatal period. Provides both individual counseling and group education that is aligned with patient household structure and cultural preferences. Skilled in working with patients and families struggling with food insecurity. Serves as a resource to the care team, members of the department of obstetrics and gynecology, and to associates engaged in delivery of services patients in this specialty area. Supports an assigned patient caseload to monitor progress, adjust management plans as needed and coordinate with the patient's medical provider and, when appropriate, other care team members such as social work.
Primary Duties and Responsibilities
Minimum Qualifications
Education
- Bachelor's degree in nutrition or any other relevant healthcare field required
Experience
- 3-4 years of progressively more responsible job related experience in diabetes education in a formal diabetes program required
Licenses and Certifications
- CDE - Certified Diabetes Educator Certification or certification eligible required
- If hired as certification eligible, employee must obtain certification within twelve months of hire date; failure to obtain certification within the twelve months will be grounds for termination
Knowledge, Skills, and Abilities
- Knowledge of diabetes theory, practice and education in order to develop educational programs, conduct patient assessments, and develop complex plans of care
This position has a hiring range of $72,758 - $130,041
