We're hiring for Clinical Pharmacist - Mercy Fort Smith

 

Job Summary:

We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our hospital team. This role works closely with physicians, nurses, and other healthcare professionals to ensure safe, effective, and evidence-based medication therapy for our patients. The Clinical Pharmacist serves as a vital drug information resource and contributes to optimal patient outcomes through collaborative care and clinical expertise.

This position performs duties in a manner consistent with our mission, values, and Mercy Service Standards.

Primary Responsibilities:
  • Review patient records to assess appropriateness of medication therapy
  • Evaluate patient conditions to ensure comprehensive treatment
  • Develop medication plans that minimize adverse effects
  • Consult with prescribers, nurses, and patients regarding medications and dosing
  • Advise on proper drug administration and monitor treatment outcomes
  • Collaborate with healthcare professionals to optimize patient care
  • Maintain accurate documentation of medication plans and patient progress
  • Routinely consult with medical staff regarding drug orders, side effects, and formulary considerations
  • Stay informed on hospital and departmental policies and procedures
  • Maintain professional relationships with medical and nursing staff
  • Attend relevant conferences and maintain licensure requirements
  • Serve as a clinical resource for care center staff with subspecialty expertise
  • Practice Medication Therapy Services including patient assessment and therapy recommendations
  • Participate in committees and department initiatives as requested
  • Ensure compliance with regulatory, licensing, and accreditation standards
  • Contribute to drug utilization evaluations and adverse drug reaction reporting
  • Support education of pharmacy students and residents
Mandatory Responsibilities:
  • Complete expiration date checks and inspections
  • Perform pharmacy and unit floor stock checks
  • Fulfill mandatory fire/safety training and continuing education requirements
Technology Proficiency:
  • Verify orders and document clinically using Epic or similar systems
  • Utilize drug information software (e.g., Micromedex)
  • Operate pharmacy automation systems (e.g., Omnicell, McKesson, ScriptPro, Baxa)
Professionalism & Service Standards:
  • Demonstrate Mercy Signature Service communication skills
  • Exhibit professionalism and courteous behavior with coworkers and patients
  • Maintain proper hygiene, PPE use, and dress code compliance
  • Work efficiently under time constraints and meet hourly deadlines
Qualifications:

Education:

  • Minimum: Bachelor of Science (BS) or Doctor of Pharmacy (PharmD) from an accredited school of pharmacy

Licensure:

  • Current pharmacist license in the state of practice

Other Requirements:

  • Strong communication, organizational, and critical thinking skills
  • Ability to work collaboratively in a fast-paced clinical environment
Preferred Qualifications:
  • Doctor of Pharmacy (PharmD)
  • PGY-1 residency
  • Two or more years of clinical pharmacy experience
  • Board certification (e.g., BCPS)
