Position:  Physical Therapist - Mercy Stoddard

 

Department: Physical Therapy Acute

 

Shift: Full-Time (40Hrs. Week), Days

Overview:

 

Under the general supervision of Therapy Leadership and according to departmental policies and procedures regarding referral, standards of treatment, performs objective assessments, as well as comprehensive individualized treatment programs for adult and geriatric patients. Responsible for direct supervision to patients, indirect supervision of rehabilitation technicians. General supervision of PT assistants in accordance with State Practice Acts. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.

 

Job Descriptions: 

 

Perform Initial Assessment/Plan of Care for each patient upon receiving physicians order according to departmental policies and procedures and standards of care.
1.1 Perform initial patient evaluation using appropriate objective tests and evaluations with no errors.
1.2 Inquire as to patient's and/or caregiver perceived goals.
1.3 Interpret objective tests, and with the patient's and/or caregiver input, establish functional treatment goals with defined time frames.
1.4 Courteously inform the patient and/or caregiver of modes to achieve perceived goals.
1.5 Collaborate with all disclipines involved in the interdisciplinary plan of care.
1.6 Develop effective age-appropriate treatment plans for patient populations.
1.7 Document initial evaluation/plan of care with clear and measurable treatment goals according to department policy.
1.8 Document initial FIM scores for IRF/PAI form according to department policy.

Administer appropriate modalities, exercise regimens, and/or treatment procedures according to departmental policies and procedures and standards of care.
2.1 Courteously explain to patient (and/or caregiver) treatment procedures before administering.
2.2 Perform individualized skilled procedures in a professional and courteous manner.
2.3.Resassess and update goals: modify treatment as indicated.
2.4.Utilize appropriate safety techniques and infection control methods according to departmental policies and procedures.
2.5 Manage patient load according to productivity measures of the department.
2.6 Communicate with referring physician as required.
2.7Collaborate with other professionals as appropriate for the treatment of all interdisciplinary patients as described in their respective plan of care and according to departmental standards of care
2.8 Identify or create and provide written home exercise instructions(s) outlining repetitions, frequency and precautions prior to discharge, as applicable.
2.9 Educate patient, family member or caregiver patients home procedures and precautions in a professional and courteous manner.
2.10 Assure competence of family/caregiver to direct home programs through training as needed.

Maintain progress notes and departmental records on each patient according to departmental policies regarding documentation and charges.
3.1 Document daily objective progress of patients responses and effects of treatment procedures in a progress note with consideration of specified treatment goals.
3.2 Document interim and discharge notes according to department policies and procedures
3.3 Document any changes in status or treatment procedures on the day in which they occur. Notify physician by written correspondence or verbal communications as indicated.
3.4 Document changes in treatment plan if one is indicated and reason for the implemented change.
3.5 Document at least weekly evaluation of progress toward goals in conjunction with weekly patient team conference meeting.
3.6 Assure all appropriate patient charges are recorded and entered by the end of the day.
3.7 Document daily FIM scores
3.8 Document all education activities

Participate in promoting education within the Rehab department.
4.1 Attend 80% of departmental inservice meetings/staff meetings and 100% of all mandatory hospital meetings.
4.2 Participate in department and hospital CQI/FOCUS activities
4.3 Accrue appropriate number of continuing education credits each monitoring period according to the requirements of supervising and licensing agencies..
4.4 Participate in hospital/department initial/annual competency requirements.
4.5 Maintain current CPR certification and state license requirements
4.6 Participate in orientation of new employees as requested by Therapy Education Coordinator.
4.7 Participate in providing in services after attending outside seminars/courses.
4.8 Supervise Physical Therapy/Physical Therapy Assistant students according to Mercy Health Center and school policies/procedur


Qualifications:

 

Education: Bachelor, Master of Science in Physical Therapy or Doctorate of Physical Therapy.

 

Licensure: Current/Temporary license of Physical Therapy in state of practice.

 

Certifications: BLS certified. Required certifications must be current before assigned to care for patients independently.

 

Preferred Experience: Prefer experience with care for adult and geriatric patients.

 

Physical Requirements:

 

•    Position requires the ability to push, pull, and/or lift 50 lbs on a regular basis.

•    Position requires prolonged standing and walking during each shift.

•    Position requires the ability to grip, reach, bend, kneel, twist, and squat to perform duties.
