Mercy Virtual is seeking Board-Certified Internal Medicine & Family Medicine Physicians to join our Advanced Primary Care team. This hybrid work-from-home/in-office position is based in St Louis, MO. Position requires minimum of five years of primary care experience. Our growing Advanced Primary Care team currently consists of 6 physicians, 35 advanced practitioners and an engaged support staff focused on quality patient care.
Mercy Advanced Primary Care is a high functioning, motivated team of clinicians and ancillary service providers that serves as the vital link bridging gaps between a patient's traditional primary care visits. Our mission is to enhance the management of chronic diseases and expand access to acute services through team-based care and innovation to provide a seamless, top-tier patient experience. Care is delivered using a collaborative care model. The physician role is to provide clinical guidance and work closely with the advanced practitioners and nurses who provide the patient care via virtual means.
Responsibilities include:
- Responsible for chronic disease management, discharge follow up, acute care as needed, advancement of GDMT and advance care planning.
- Help drive medical management, improve quality, and reduce total cost of care for Medicare and Medicare Advantage patients.
- Provide education, mentorship, and development of the APC clinical team
- Assist PCP offices with panel manageme nt through identification and clinical follow up of identified patients.
- Coordinate care with patient's multidisciplinary care team including primary care physicians, specialists, social workers, physical therapists, palliative care, etc.

The virtual care program provides care for patients in seven states, combining caregivers and technology to extend Mercy's reach and services well beyond the walls of doctors' offices, hospital campuses and other traditional facilities.
