After successful completion of the probationary period, employee will receive off probation wage increase.


Summary:

Provide a variety of well-balanced and quality services to include preventative, rehabilitative, transport, home health and liaison and advocacy services to tribal members of the Oneida community under the direction and supervision of an RN Case Manager.


Responsibilities:

The Community Health Representative position performs the following essential functions within the framework of Public Health Core Competencies (PHCC) and Home Health Regulations. The duties and responsibilities will be subject to change based on organizational needs and/or deemed necessary by the program manager. Staff participate in assessing and evaluating organizational needs to meet the needs of the Oneida Community.

 

Data Analytics and Assessment Skills

The ability to understand factors affecting the health of Oneida and surrounding communities as it relates to community members that may have lack of transportation, have lower income, have increased need for health information and health services, or increased likelihood to have chronic diseases.
Ensure quality of services by performing routine monitoring visits to assigned clients.
 
Policy Development and Program Planning Skills
3.  Participate in the development of departmental goals and objectives in alignment with organizational goals objectives, and strategic plans for the Health Division and the Oneida Nation.
4. Follow program policies and contribute to updating program policies that are consistent with all applicable laws and accreditation regulations.
5. Participate in ongoing strategies for continuous quality improvement of the department and Health Division.
6. Ensure program compliance by completing necessary reports as required.  
Communication Skills
7. Ability to inform and communicate verbally and in writing with team members and in electronic medical record for patient care.
8. Ability to communicate information to educate clients on health issues to improve health.
9. Ability to operate business computers and office equipment, including in a Windows environment, specifically Word, Excel, SharePoint, Teams, and Presentation software.
10. Provide outreach-liaison services for Oneida Community Health Center programs and services. Serve as liaison between clients/families and medical/nursing care providers.
11. Communicate among team members to ensure the appropriate care.
12. Attend multi-disciplinary treatment meetings when necessary.

Health Equity Skills

13. Interact with customers, community partners and co-workers with fairness and equity and deliver services free of bias and prejudice.
14. Ability to demonstrate the value and take measures to support a diverse public health workforce.
15. Support and encourage the development and incorporate Oneida Nation culture, tradition, and values into program practices.
16. Provide transportation services to community members per program guidelines.
17. Pick up and deliver medical equipment, medications, materials, or supplies to patients' homes

Community Partnership Skills
18. Serve on committees involved in improved patient outcomes to coordinate health self-care activities.
19. Collaborate with peers and RN for promotion of wellness and disease prevention.
20. Provide home bound patients with in-home personal care, health monitoring (Vitals & behaviors) and home chore tasks as directed and supervised by a RN Case Manager.
21. Report any child abuse or domestic violence situations.
22. Report any adult at risk and elder abuse situations.
23. Participate in Community outreach screening programs under the direction of Community Health Manager.
24. Assist with immunization programs to include entering data on computer or in logbook, assisting with clinics and verifying information for accuracy.

Public Health Sciences Skills
25. Use evidenced based and best practices in programming as determined by program leaders.
26. Provide case management duties to patients as designated by RN Case Manager. 
Management and Finance Skills
27. Complete appropriate documentation for patient care services and reports to support any available funding reimbursement.

Leadership and Systems Thinking Skills
28. Network with other community programs and departments providing community services.
29. Encourage area residents to utilize services provided by Oneida Community Health Center.
30. Contribute to a team effort and accomplish related results as required.

Certified Nursing Assistant Skills
31. Provide hands on personal care to include:
              a. Baths
              b. Back Rubs
              c. Oral hygiene
              d. Shampoo Hair
              e. Change bed linen.
              f. Assisting patients with dressing and undressing
              g.Skin care to prevent breakdown.
              h. Assisting patients with toileting activities
              l. Keeping patients living area clean and orderly, as appropriate

32. Planning and preparing nutritious meals.
33. Assisting in feeding the patient, if necessary
34. Taking and recording oral rectal or axillary temperatures, pulse respiration and blood pressure when ordered.
35. Assist patient with plan of care activities to include:

                  a. Ambulation
                  b. Exercises and or Range of motion 
                  c. Self – administration of medication
                  d.  Doing patients laundry when appropriate.

36. Meeting safety needs of patients and using equipment safely and properly.
