Summary:

Perform and/or oversee a variety of administrative, patient intake, software, data collection, data presentation and staff support duties for Community Health Services Home Health and Case Management programs. Some tasks require advanced or specialized skills and knowledge of organization policies, procedures, program specific regulation gathering of information and internal and external software solutions. Continuation of this position is contingent upon funding allocations.

Responsibilities:

The Home Health Coordinator the following essential functions within the framework of the Public Health Core Competencies (PHCC). These duties and responsibilities are not an all-inclusive list but rather a general representation of the duties and responsibilities associated with this position. The duties and responsibilities will be subject to change based on organizational needs and/or deemed necessary by the supervisor.

Data Analytics and Assessment Skills

  1. Describe factors that affect the health of the Oneida Community such as income, education resilience, homelessness, access to food, access to health care etc.
  2. Assess community health status by identifying health needs in the community such as housing, transportation, food, chronic disease, immunizations, and tobacco use.
  3. Ability to collect qualitative, quantitative data in the promotion of health and wellness.

Policy Development and Program Planning Skills
  1. Implement policies, programs and services while collaborating with others.
  2. Connect individuals to prams and services by helping individuals navigate systems with in and outside the organization such as linking individuals to health care and social services.
  3. Engages in continuous quality improvement of policies, programs, services and organizational performance.
  4. Participates in strategic planning events and meetings that influence Home Care and Team building related programs and services.
  5. Organize office operations and procedures to ensure standards are met.
 
Communication Skills
  1. Contributes to the development of Community and Home Care messaging for disseminating health programs and information.
  2. Communicates information to influence behavior & improve health such as chronic disease self-care, preventive care, preparing for emergencies and discouraging tobacco use.
  3. Suggests approaches for responding to programming needs.
  4. Facilitates communication among individuals and groups by facilitating communication through building trust, active listening, participate in meetings, be clear and transparent.
  5. Operate business phone with professional demeanor.
  6. Schedule Home Health or Case Management patient or programming appointments as needed and/or requested.
  7. Manage correspondence by reviewing, sorting, logging, and responding or routing as needed.
  8. Operate business computers and office machines, including in a Windows environment, specifically Microsoft programs, and patient electronic medical record software.
  9. Ability to inform and communicate verbally and in writing in English in diverse and challenging situations with the ability to process information effectively, identify and define problems and make objective decisions.
  10. Carry out instructions furnished in verbal or written format.
  11. Participate in multi-disciplinary team setting to include one on one & group networking and coordination of patient care and programming.
  12. Thorough knowledge of Division Electronic Medical Record and department electronic medical records to retrieve needed patient information for programming and customer needs.

  1. Actively participate in performance management to include Quality Assurance and Process Improvement (QAPI) activities to include data collection and writing of reports.
  2. Responsible for the internal receiving, distribution and inventory of supplies and goods to include development of processes and procedures.
  3. Complete Home Health & Case Management Intake to ensure standards are met.
  4. Actively support the department and Division mission, vision, and strategic plan goals and objectives

Leadership and Systems Thinking Skills
  1. Maintain and operate office machines, equipment, and computers. Perform or coordinate general maintenance and repair.
  2. Ability to be the superuser of software programs utilized by department and be available to staff to educate and problem solve software functionality.
  3. Conduct chart audits and maintain electronic health record files for completeness and accuracy.
  4. Conduct health outreach campaigns related to Home Care programs offered by the department.
  5. Conduct research to resolve operational questions or issues; make recommendations to enhance the efficiency of Home Care programs and software operations.
  6. Effectively handle multiple tasks and meet deadlines.
  7. Contributes to team efforts to accomplish results as required.
  8. Work both independently and in a team environment.
  9. Ability to work responsibly and effectively with minimal supervision or in a team environment.
  10. Willing to obtain additional education and training.
  11. Offer creativity and innovation in addressing and responding to emerging program and customer needs.
  12. Assist with Administrative and Office Coordinator task as needed.
