RN Intake and Triage Coordinator – Medical Oncology
Location: Pewaukee Cancer Center
FTE: 1.0 (Full Time, Days)
Schedule: Monday–Friday, 8:00 am–4:30 pm (may flex to 5:00 pm)

Join our dedicated cancer care team at UW Health at ProHealth Care Cancer Center and make a lasting impact from a patient's very first step into oncology care. We're seeking an experienced and compassionate RN Intake and Triage Coordinator to serve as a clinical liaison and care guide for new patients referred to our Oncology program.

In this unique role, you will be the RN managing intake and triage for new oncology consults. You'll collaborate closely with physicians, APPs, and scheduling staff to ensure each patient is thoughtfully prepared for their first visit and receives timely, appropriate care. You'll also support patients over the phone and in person, creating a trusting relationship that enhances their entire care journey.

What You'll Do:

  • Review and triage new oncology referrals, determining urgency and readiness for consultation

  • Coordinate with referring providers to ensure complete and timely documentation

  • Contact patients before their visit to introduce yourself, offer guidance, and gather needed information

  • Collaborate with cancer care teams to schedule the right services at the right time

  • Meet patients at their first consult to provide support and clarity, and follow up as needed

  • Serve as a resource for patient education and support, using motivational interviewing to promote engagement

  • Identify and help overcome barriers to care, including emotional and logistical challenges

  • Document in Epic accurately and efficiently to support care coordination and compliance

Qualifications:

  • Current RN license in Wisconsin required

  • Associate degree in nursing required; BSN preferred

  • Minimum 2 years of RN experience, preferably with complex or specialty patients

  • Strong comfort with Epic, Microsoft Office, and digital documentation systems

  • Self-motivated, independent, and organized with excellent communication skills

What You Will Do:

The RN Intake & Triage Coordinator is a vital member of the Medical Oncology team at the UW Health at ProHealth Care Cancer Center locations. This individual is responsible for the following: 


 • Initiates, monitors and evaluates best practices for new patient referrals to Hematology/Oncology.
• Collaborates with physicians, advanced practice providers, and ancillary health care team members to coordinate system resources to maximize appointment efficiency for new patients.
• Advocates for the patient by providing support and resources in order to reach optimal hematology/oncology care
• Develops a patient-centered plan of efficient care, operating from an in depth understanding of their unique meaning of health, illness and disease
• Triages patient calls to the appropriate service (i.e.  Triage Bay, ED, appointment).
 
The responsibilities of this nurse include: 

• Assists the patient in getting appropriate appointments to address their unique diagnosis.
• Coordinates appropriate referral information from the referring provider.
• Determines patient acuity/urgency based on diagnosis and medical record clinical information.
• Facilitates patient access to the appropriate Cancer Care team
• Promotes stress management by helping the patient and family cope with the fears and concerns that accompany life and health changes
• Provides evidence-based patient education and assists in shared decision-making
• Provides care coordination and collaboration during patient transitions across the Continuum of Care
• Identifies barriers to disease self-management and self-care
• Functions independently and possesses the internal discipline/drive to guide care for patients with complex health needs
• Completes all EHR documentation accurately, timely and efficiently for regulatory compliance
• Demonstrates effective use of healthcare and information technology resources
• Attends appropriate educational opportunities and shares knowledge and experiences with others in formal and informal settings. Maximizes personal growth and develops an individual educational plan.

You Will Need:

  • Registered Nurse (RN), WI or eNLC State
  • Associates degree in nursing required, bachelors preferred
  • Minimum of 2 Years experience in nursing
  • Basic understanding of computer use, Microsoft Outlook, internet and health information technology (Epic EHR required).
  • Proficient in Epic EHR, Microsoft Office suite and other health information technology.
