RVCC is looking to hire a Support Specialist for the Medical Case Management team! River Valley Counseling Center, Inc#s Mission is to improve the health of all people in our community. We do that through expert and compassionate care, education, community partnerships, fostering innovation, and by inspiring hope in all we touch.# We do that by being good stewards of our resources and providing efficient and cost-effective care for all. # Overview of Job Responsibilities: Supports Medical Case Management#clients with HDAP application in order to maintain their medications Follows up with HDAP Program re: status of applications and submitting any necessary documents Supports medical case managers with obtaining ct medical records every six months through HIV medical providers. Creates and maintains a tracking chart of medical records requests in order to compile data related to turnover time for records requests Responsible for scanning received medical records into clients EHR document library Responsible for supporting the Assistant Program Director with monitoring referrals and following up with a waitlist to identify if clients are still interested in services Processing new client intakes by obtaining required documentation to ensure eligibility for the medical case management program Support Assistant Program Director with identifying and scheduling client educational events related to HIV education and support Support Assistant Program Director with Careware data entry and running reports Job Requirements: Education Level -#High School Diploma Years of Experience -#1 year of administrative experience preferred Licensing -#Valid Driver#s License and reliable transportation as this position may require outreach Skills/Abilities -#Bilingual (English/Spanish) preferred but not required. Excellent verbal and written communication skills; interpersonal skills; crisis intervention skills; organizational skills; attention to detail; computer skills; good customer relations skills; knowledge of legal and ethical issues pertaining to the profession. Experience using excel, google sheets and must have data entry experience.
