Schedule: Full time, days
Job Specific Competencies:
Preventive & Responsive Care
- Support families and providers by promoting the HealthySteps model.
- Participate in team‑based well‑child visits focused on development, behavior, and family support.
- Provide short‑term consultations on common developmental and behavioral concerns (sleep, feeding, parenting, etc.).
- Maintain a family support line and manage follow‑up in a timely manner.
- May conduct home visits, accompany families to appointments, or facilitate caregiver groups.
Referrals & Community Collaboration
- Maintain an up‑to‑date community resource database and track referrals.
- Build relationships with community partners to support warm handoffs and appropriate referral pathways.
- Participate in relevant early childhood or mental health community meetings.
Team-Based Care
- Actively engage with the HealthySteps Implementation Team.
- Coordinate scheduling with administrative staff to align with well‑child visits.
- Support Continuous Quality Improvement and implementation of HealthySteps core components.
- Communicate effectively with the primary care team and provide guidance on child development and early relational health.
Caseload Management & Documentation
- Maintain databases to meet HealthySteps reporting and fidelity standards.
- Document all clinical activity in the EHR and track caseload to ensure appropriate service level and care transitions.
- Assist with annual site reporting and data analysis.
Professional Development
- Participate in required HealthySteps trainings and annual refreshers.
- Attend supervision meetings and complete annual competency-based development plans.
Education and Experience:
- Required: Clinically licensed Masters in social work, psychology, or childhood education/developmental fields
- Experience and knowledge about early childhood growth and development, parent-child relational health, infant and early childhood mental health, and family systems
Pay Range: $28.00 - $31.00
Starting Pay: Based on experience
