Description
Patient Care (Outpatient/Inpatient):
- Perform comprehensive patient assessments, including detailed history taking and physical examinations tailored to nephrology.
- Develop individualized treatment plans for kidney-related conditions.
- Diagnose and manage acute and chronic nephrological conditions such as CKD, hypertension, and electrolyte imbalances.
- Prescribe medications and treatments, prioritizing renal health.
- Educate patients on renal health promotion, disease prevention, and CKD management.
- Coordinate kidney biopsies, ensuring proper scheduling and patient preparation.
Collaboration and Communication:
- Collaborate with nephrologists and healthcare providers to ensure optimal patient outcomes.
- Participate in interdisciplinary team meetings and care coordination activities.
- Communicate effectively with patients and families regarding diagnosis, treatment options, and follow-up care.
Quality and Safety:
- Adhere to clinical guidelines and evidence-based practices in nephrology.
- Participate in quality improvement initiatives and patient safety programs.
- Ensure compliance with regulatory and accreditation standards in nephrology care.
Professional Development:
- Engage in continuing education related to nephrology.
- Maintain necessary certifications and licensure.
- Participate in peer review and performance evaluation processes.
- May be required to cover weekday inpatient consults at MTC when necessary.
Administrative Duties:
- Document patient encounters accurately in the electronic medical record system.
- Assist in clinical lab and test follow-up, ensuring timely review of critical results.
- Support with prior authorizations and administrative tasks.
- Contribute to practice management and strategic planning.
Addressing Nephrology Challenges:
- Manage outpatient nephrology patient volume efficiently.
- Ensure timely follow-up on test results and patient outreach.
- Undertake tasks beyond Medical Assistants' scope to enhance patient care.
EDUCATION and/or EXPERIENCE
- Master of Science degree with completion of a Nurse Practitioner program.
- Current certification as a Nurse Practitioner, with a nephrology specialty preferred.
- Registered and currently recorded with the Maryland Board of Examiners of Nurses and as a Nurse Practitioner in the State of Maryland.
- Current certification as a Nurse Practitioner, with a nephrology specialty preferred.
- DEA and CDS eligible.
- CPR required.
- ACLS as required by needs of the department
- A minimum of one year experience as a Registered Nurse is required.
