Description

Provides short-term counseling, assessment and referrals for clients of the EAP. Conducts supervisor information and training sessions. Consults with supervisors, managers and HR staff. Serves as a lead worker to other EAP Counselors. May serve as back-up for EAP Manger and EAP Director in supervising the office; and other duties as assigned.

ESSENTIAL FUNCTIONS

  • Consults and collaborates with medical providers and other team members regarding the
  • diagnosis and treatment of behavioral health and other psychosocial issues. Assists in effectively managing, treating and counseling patients with chronic mental health conditions.
  • Conducts psychosocial assessments, screenings and triage of patients, as needed.
  • Develops individualized care plans for patients based on needs assessments which identify problems, goals, skills training, and other service needs.
  • Provides support to patients to assist them in dealing with social, emotional, financial and or/environmental factors which affect their well-being.
  • Provides short term individual and group counseling, crisis intervention, as well as education and skills training.
  • Assists in the detection of high-risk patients to assess and provide care to prevent further psychological deterioration.


EDUCATION and/or EXPERIENCE

* Master's in social work or related field
* Excellent interpersonal and team leadership skills
* Excellent clinical skills in mental health and addictions treatment
* Excellent written and verbal communication skills
* Ten or more years of experience
* Licensed Professional Counselor (LPC) and Certified Employee Assistance Professional (CEAP) highly recommended
